Services Peace Hospice Offers
At Peace, your care team will consist of you, your family and close friends, your physician and/or a hospice physician, hospice nurse and hospice aide/homemaker, social worker, spiritual and bereavement counselors, as well as hospice volunteers. Others who may be involved in your care include but are not limited to dietician, physical, speech and occupational therapists, music therapist, and pharmacist.
The primary physician is the patient’s personal doctor. He / She maintains a leading role and is a key member of the hospice team. They are responsible for the supervision and approval of the patient’s care plan.
Hospice Medical Director
The hospice medical director works hand in hand with the primary physician to supervise and approve the patient’s care plan. He/She may also serve as a patient’s primary physician when necessary.
Registered Nurse / Licensed Practical Nurse
The hospice RN is the patient’s case manager. The hospice RN and LPN work alongside the patient’s doctor, suggesting solutions regarding pain and symptom management. They are responsible for the patient’s general physical comfort and coordination of the patient’s care with the interdisciplinary team.
Certified Nursing Assistant/Homemaker
The CNA is supervised by the RN and is responsible for assisting with the patient’s personal care. Homemaker provides help with personal needs which may include light housekeeping.
Hospice Chaplain / Prayer Line
The hospice chaplain provides spiritual support to every patient and their family members requesting such. The Hospice Chaplain serves as the bridge between patient/family and their usual place of worship. They can also help to find a clergy of your faith. At Peace, our patients and families have access to our 24-hour prayer line.
The hospice social worker provides overall support for the patient and their family. They can assist with financial planning, advance directives as well as counseling and emotional support.
The hospice volunteer plays a very special role in the plan of care. They are trained to provide a special kind of caring, companionship and respite for our patients and their families. They may also provide emotional and bereavement support, office help and assistance in many other ways.
Medications / Medical Supplies / Medical Equipment
A contracted vendor normally provides medical equipment and medication directly related to your hospice diagnosis.
Bereavement / Grief Support
Additional support and comprehensive bereavement services are available for the families of hospice patients, as well as community members, following the loss of a loved one. Support may include home visits by staff and/or volunteers, mailings and phone calls. Bereavement support is available for up to 13 months after the loss.
Music can truly bring peace. A music therapy session with a Peace Hospice patient is a goal-directed therapy with a person-centered approach. The music therapist uses music to address and improve the patient’s quality of life.
Dietitian, Speech, Physical and Occupational Therapists
Therapists are utilized, as needed, to improve our patients’ quality of life.
Peace Hospice is available to serve you all day, every day, all year long. Our prayer line is also available to you and your loved ones.
Peace Hospice acknowledges that every patient’s experience is different. We, therefore, personalize the care plan according to patient and family needs. Additionally, our Inter-Disciplinary Team (IDT) regularly review these plans to initiate and effect changes as necessary.
